CHS supports Miami Dade Corrections & Rehabilitation at 3 facilities in Miami-Dade County and leverages the resources of a large public safety net health system in the areas of education, quality, risk management & compliance.SummaryClinical Practice Pharmacist performs a variety of pharmacy procedure requiring professional knowledge and skills in a large and complex department of pharmacy. Emphasis of the work is on compounding and dispensing a variety of medications, manufacturing of more complex solutions and in supervising work of pharmacists not registered within the state. All work is performed in accordance with recognized professional standards, hospital and pharmacy rules and regulations and applicable state and federal laws.ResponsibilitiesIntegrates drug distribution and clinical pharmacy services in the pharmaceutical care model in order to assure safety and efficacy, and to reduce costs. Evaluates physicians orders, prescriptions, vouchers, and cart fill lists for appropriateness of therapy, taking into account drug interactions, allergies, and relevant patient conditions. Assists in the collection of data for drug utilization evaluations. Assists in the collection of data for drug utilization evaluations. Provides drug therapy monitoring and intervention through target drug programs depending on area assignment. Serves as a role model and educator for pharmacy students performing internships and externships, and pharmacy residents at JHS as required. Actively participates in staff development programs, providing programs when requested. Provides therapeutic drug level monitoring (e.g. recognizes need for levels, schedules and interprets levels).
